West Berkshire Council is recommissioning the NHS Health Checks Service: Primary Care Pathway for eligible West Berkshire residents aged 40 to 74. The service will include all elements of the NHS Health Check pathway, including identification of the eligible population, risk stratification, call and recall, delivery of health checks, cardiovascular risk assessment, personalised risk communication, behaviour change support, onward referral, clinical follow-up, data recording and reporting. The service is intended to improve access to preventative interventions, identify cardiovascular risk and reduce health inequalities.
